Commonwealth Home Support Programme

The Commonwealth Home Support Programme (CHSP) provides entry-level support services to seniors, helping them to live independently and safely at home.

What supports can be provided?

A variety of supports are available, depending on your assessed needs:

  • Personal care services e.g. support with showering and personal grooming
  • Social support e.g. attending social groups and activities or one to one supports
  • Home maintenance e.g. gardening
  • Allied health support e.g. physiotherapy and podiatry
  • Nursing care e.g. wound care, medication assistance and health education
  • Aids and equipment e.g. mobility aids
  • Transportation
  • Domestic assistance e.g. cleaning, grocery shopping and meal preparation

 

Steps to access the Commonwealth Home Support Programme:

  1. Before you can access government-subsidised aged care services for the first time, you will need to apply for an assessment.


    Contact     My Aged Care online or call 1800 200 422. This assessment is free.

  2. After your assessment is complete, you will receive a referral code for the services you are eligible for.

  3. Contact the Rural City of Wangaratta Aged Care team on 5722 0790.

  

Programme Fees

This programme is subsidised. The amount you need to contribute will depend on your income.

We'll make sure to go over any costs with you before we begin providing services.

These income ranges are relevant to Rural City of Wangaratta Aged and Community Care Fees Policy and are for gross income only.
