Change name or address

Step 1.Gather your documents

To change your details, you'll need to submit the following documents:

• Change name: Completed Change of Name form(PDF, 299KB) plus a copy of supporting documentation (marriage certificate, birth certificate, change of name certificate, decree nisi)
• Change residential address: Completed Change of Residential Address form(PDF, 173KB)
• Change business address: Completed Change of Business Address form(PDF, 168KB) plus a copy of your business registration and a copy of your ACN, ABN or ASIC print out.
