Home & Community Care for Young People

The Home and Community Care Program for Younger People (HACCPYP) provides basic support services to help people with disabilities to remain living independently at home.

Services are targeted to younger people who need assistance with daily living tasks due to an impairment, mental or chronic illness and their carers. These services aim to help with daily activities. HACCPYP is an alterative option for you if you are not eligible for the National Disability Insurance Scheme (NDIS). 


To be eligible for this program, you must be aged under 65 years old with a disability (under 50 years for Aboriginal and Torres Strait Islanders).

To access these services, you need to be assessed and approved as eligible by an Assessment Officer. The Assessment Officer will visit you to talk about your needs and establish the level of support you need. They will develop and agree an individual care plan with you.

This program is subsidised by the Victorian Department of Families, Fairness and Housing and delivered by Rural City of Wangaratta. However you will be required to contribute towards the cost. The amount you need to contribute depends on your income.
