The Home and Community Care Program for Younger People provides basic support services to help people with disabilities to remain living independently at home.
Services are targeted to younger people with moderate, severe or profound disabilities and their unpaid carers. These services aim to help with daily activities. You can find a list of available services further below.
Eligibility
To be eligible for this program, you must be aged under 65 years old with a disability (under 50 years for Aboriginal and Torres Strait Islanders).
To access these services, you need to be assessed and approved as eligible by an Assessment Officer. The Assessment Officer will visit you to talk about your needs and establish the level of support you need. They will develop and agree an individual care plan with you.

Fees
This program is subsidised by the Victorian Department of Health and Human Services and the Rural City of Wangaratta. However you will be required to contribute towards the cost. The amount you need to contribute depends on your income.
