Case Management and Brokerage Services
Case management is provided to residents of the following municipalities - Rural City of Wangaratta, Rural City of Benalla, Mansfield Shire, Moira Shire, Strathbogie Shire, Towong Shire, Alpine Shire, Indigo Shire and the City of Wodonga.
Case management, service co-ordination and brokerage is provided for older people, people with disabilities who have complex needs and their carers, who require assistance to remain living independently in the community.

Package Care is the co-ordination of support that is delivered to you in your home, with the aim of enhancing your independent living as long as possible. Package care is available to people with disabilities, frail and older people, people with acquired brain injury, people with dementia and people with complex care needs.
